Collaborative Documentation
As advocates, we are committed to promoting family-driven care and authentic partnership. Families should be meaningfully included in every aspect of the services provided — including documentation. This Technical Assistance training will explore the practice of collaborative documentation and how it strengthens engagement, transparency, and shared advocacy. Participants will learn practical strategies to develop a shared language with families, promote empowerment through inclusive documentation, and engage families in writing their own progress notes. We will also review how to write clear, concise notes that follow the “golden thread” from goals to interventions to outcomes. Join us to strengthen your documentation practices while shifting from writing notes for families to writing notes with families — reinforcing partnership, voice, and shared accountability.

Affinity space for credentialed Family Peer Advocates to promote collaboration, skills development, inclusivity and belonging.
